You will be asked to sign a COVID-19 consent before your procedure (if you would like a copy prior to your appointment, please call to request)
If you have any symptoms of illness (cough, temperature of 99.0 F or higher, body aches, etc.) notify our office prior to your appointment!
Please be advised, we are following the Governor’s Safer At Home Guidelines and DORA’s regulations. If your appointment is something that we can help you with virtually or over the phone, we encourage you to do so!
